Upload Request Form

In an effort to better serve you and track our issues, the Web Services Team is launching a new Ticketing System to handle support requests.  All requests which normally would be sent to the Web Services email address should now be entered into our Ticketing System, which is located at http://sqlsh01/otrs/customer.pl .  You will need to be on the network to access this site, so if you are at home you need to be on the URMC VPN.  Click here for information about connecting to the VPN
 
You can log in with your regular domain username and password, and all your requests will be tied to you.

• Please list the files or folders changed, their complete destination path (one file per line) and any instructions:

e.g.,

1. /hslt/miner/services/index.html
2. /smd/mdphd/index.html (new file)
3. /smd/mdphd/test.html (delete file)
4. /smd/mdphd (upload complete folder)
5. /smd/images/imagename.jpg
