Stroke Rehabilitation Program

Strong Memorial Hospital offers a rehabilitation team that works with stroke survivors beginning in the acute phase of stroke recovery. The team works with patients to ensure their early discharge to the most appropriate location, whether to home, a skilled nursing facility, or a rehabilitation facility. Our interdisciplinary team works closely with stroke survivors and family members to improve care.

A stroke rehabilitation program is individually designed to meet a person’s specific needs. A multidisciplinary team approach for care and service is the basis of stroke rehabilitation treatment. Multidisciplinary refers to the fact that many different disciplines work together toward a common goal—improving health.

The rehab team is usually directed by a physiatrist (physician specializing in rehabilitation), with other specialists playing important roles in the treatment and education process. Team members involved depend on many factors, including patient need, facility resources, and insurance coverage for services. The American Stroke Association emphasizes the need to take steps to adapt the way you live, with the help of rehabilitative services, after a stroke.
