Welcome to the Geriatric Medicine Fellowship Program at the University of Rochester. We offer a one-year ACGME accredited training program to board-eligible internists and family practitioners. Additional years of  research-based training are available to qualified applicants via the T32 Research Training Grant. The fellowship is designed to fully meet the requirements of the Accreditation Council for Graduate Medical Education (ACGME) and to abide by the guidelines of the American Geriatrics Society.The fellowship is an integrated experience based at several sites: Monroe Community Hospital (teaching nursing home), Highland Hospital - Acute, Independent Living for Seniors (a PACE replication site), and the Veterans Administration Medical Center in Canandaigua and Rochester, New York.

The experience in year one is clinically focused and includes longitudinal care and neurology consultation in a nursing home setting, primary care of a cohort of frail, older adults, and comprehensive geriatric assessment at the consultative clinic at Monroe Community Hospital. geriatric psychiatry, rehabilitation, acute inpatient care, palliative care, Independent Living for Seniors, and home care.  Fellows also participate in the educational activities of the Division including: geriatric journal club, geriatric medicine grand rounds, geriatric fellows' conference and as well as participating in the education of internal medicine housestaff and medical students.

It is our goal that by the end of twelve months of sub-specialty training the geriatric medicine fellow is fully prepared to provide primary and consultative health care to geriatric patients in both inpatient and outpatient settings. Each fellow will have at his/her command a broad body of knowledge that includes an integrative understanding of normal aging and disease models, training in biological and psychosocial aspects of normal and abnormal aging, the impact of acute and chronic physical illnesses, and the biological and psychosocial aspects of the pathology of geriatric syndromes beginning in or continuing into older age.

It is also our goal that the fellows’ training includes a multi-dimensional biopsychosocial view of assessment, treatment, and management of geriatric patients in all settings.  Fellows will gain skills through a combination of supervised clinical experiences and formal didactic conferences.  In addition to the requisite body of knowledge, each fellow develops leadership and interpersonal skills, professional attitudes, and practical experiences required of a geriatrician. Specific learning objectives are provided for each clinical rotation in which the fellow participates.

Geriatric Medicine Fellowship Clinical Training Program

The Geriatric Medicine Fellowship is a one-year ACGME accredited training program for board-eligible internists and family practitioners. The fellowship is an integrated experience based at several sites: a teaching nursing home, an Acute Care for Elders Unit (ACE) in a community hospital, a Program for All Inclusive Care for the Elderly (PACE) site, and a Veterans Administration Medical Center (VAMC). The fellowship is clinically focused and includes longitudinal nursing home care, rehabilitation, neurology consultation, primary care of outpatients, home care, and comprehensive geriatric and psychiatric assessment. Fellows also participate in Geriatric Division’s educational activities including: journal club, grand rounds, and fellows' conference, as well as participating in the education of internal medicine housestaff and medical students.

At the end of twelve months of sub-specialty training the geriatric medicine, fellows are eligible for subspecialty board certification in geriatric medicine. Interdisciplinary Geriatric Fellowship – Medicine, Psychiatry, and Dentistry

The Interdisciplinary Geriatric Fellowship is a program designed for medical, dental, and psychiatry trainees with a career goal of becoming an effective clinician educator. This two-year program provides each fellow with traditional clinical experiences in their respective disciplines, while also enhancing interdisciplinary experiences through common core clinical, didactic, research, and administrative activities.

Geriatric psychiatry and medicine fellows completing the two-year fellowship will be eligible for subspecialty geriatric board certification. Clinical experiences include unique interdisciplinary geriatric assessment clinics targeting underserved populations. Fellows also participate in an innovative Academic Career Development Course, developed in conjunction with the University of Rochester’s Warner Graduate School of Education and Human Development, with the goal of preparing the next generation of geriatric academic leaders.

The course curriculum provides trainees with the skills to successfully develop, implement, and evaluate academic geriatric programs; learn the necessary teaching and leadership skills; integrate public health precepts into quality improvement initiatives; and relate more effectively with patients, families, and professional colleagues.

Research Training in Geriatrics and Gerontology

Sponsored by the National Institute on Aging (NIA)

 

The Research Training in Geriatrics and Gerontology program is a post-doctoral (M.D. or Ph.D.) grant designed to produce independent academic investigators in clinical and health services research related to older adults. Trainees are accepted for a two-year, NIA funded (T32) interdisciplinary program that combines research coursework with a mentored research experience. Special preference is given to individuals who have completed a one-year Clinical Geriatrics Fellowship.

Trainees complete a series of didactic courses aimed at acquiring the knowledge necessary for conducting clinical research, including biostatistics, epidemiology, and design of clinical trials. In addition trainees complete a series of workshops to build research skills, including research ethics, scientific communication, medical informatics, and grant writing.

In planning their mentored research project, trainees will work with a mentorial panel, comprised of one Primary Research Mentor and one or more Geriatrics/Gerontology Faculty Advisors. Mentors will be chosen from 5 research “clusters”: cardiology, geriatric health services, neuropsychiatry, oncology, and infectious diseases.

Trainees will be expected to write a career development grant by the end of their training period, to assure that they have protected time as they transition to being a junior faculty member.

Training and Rotation Sites

Monroe Community Hospital

MCH is a 566-bed residential health care facility providing quality long-term care to individuals with complex or chronic health conditions.

1. Geriatric Assessment Clinic – This clinic was developed at Monroe Community Hospital to address the long-term health concerns faced by older individuals and their families. This comprehensive, interdisciplinary outpatient consultative program was one of the first of its kind in the nation, and has gained national recognition since its inception in 1980. Fellows participate in the clinic regularly throughout their fellowship and provide the medical assessment and consultative liaison service for the patients seen in the clinic. Fellows work with a team of health professionals including an attending physician, registered nurse, neuropsychologist, and social worker to thoroughly assess the health and functional ability of each individual patient by conducting a series of examinations and visits. This comprehensive process allows the team to fully understand the health care needs of each individual and their families in order to prepare a detailed customized care plan.

Training2. Long-Term Care - Each fellow is assigned a panel of long-term care patients, at Monroe Community Hospital, which they will follow longitudinally with an attending physician during their tenure as a fellow. Other experiences include participation on a closed wandering/behavior unit at MCH and a respiratory unit for ventilator-dependent patients at an affiliated facility.

3. Neurology - Fellows participate in inpatient neurologic consultation at Monroe Community Hospital and outpatient consultation and neurology clinic at Monroe Community Hospital.  They are involved in didactic teaching rounds with an attending geriatric neurologist.

4. Psychiatry - The Psychiatric Department at Monroe Community Hospital is world-renowned for its groundbreaking work in cognitive disorders and dementias and continues to actively participate in innovative clinical trials. Fellows participate in a two month rotation of inpatient psychiatric consultation on long-term care patients at Monroe Community Hospital as well as outpatient evaluation in the neurology and psychiatry clinic. (Their involvement is also encouraged in the ongoing research and clinical trials in Alzheimer's Disease and other dementing disorders and behavioral disturbances.)

Training5. Rehabilitation - MCH is home to one of the most experienced short-term rehabilitation programs in the region providing individualized care to people with disabling conditions as result of an illness or injury. Geriatric fellows participate in a three month rotation on a comprehensive, interdisciplinary teaching service and are responsible as team members for the primary care of the patients. Fellows interact with specialists in medicine, physical therapy, occupational therapy, speech and language pathology, recreation therapy, social work, clergy, audiology, nursing, and nutrition. The treatment team is committed to restoring each patient's physical and cognitive skills, enabling him or her to be as self-reliant and independent as possible.

Highland Hospital - Acute Care for Elders (ACE) unit. Highland Hospital Highlandis
a community hospital that has a long tradition of outstanding care with research and education in the biopsychosocial model and physician-patient communication. It is quickly becoming the center for the care of the hospitalized elderly with its innovative ACE Unit. Fellows have an opportunity to both learn and teach during this required rotation. Fellows also participate in peri-operative geriatric consultation and comanagement on the Geriatric Fracture Center and Palliative Care Consultation.

Independent Living for Seniors (ILS ) - a PACE (Program for All-inclusive Care for the Elderly) replication site. A PACE site provides an interdisciplinary, adult daycare centered comprehensive plan of care to nursing home eligible individuals in their own home. Most disciplines including physicians, nurse practitioners, skilled nursing, rehabilitative services, social work, pastoral care, dental care, eye care, podiatry, dietary, and aide services are offered at the three day centers.

The Department of Veterans Affairs has a long history of supporting geriatrics educational and research activities. The Canandaigua VA Medical Center is a 300-bed facility affiliated with the University of Rochester which provides extended care for veterans with physical and mental disabilities. The 150-bed nursing home program includes post-acute care, medical nursing home care and a 30-bed dementia unit. The Canandaigua facility and an affiliated Rochester clinic located proximal to the University and Monroe Community Hospital, also offer a variety of outpatient services. Fellows rotating through the VA have opportunities for involvement in the outpatient clinic services, palliative care services and the Home Based Primary Care programs.

The fellowship also utilizes several other sites as part of the training including Visiting Nurse Services and Highland House Calls for the home care rotation and palliative care rotation; Lifetime Health and Unity Geriatrics Associates for primary care ambulatory clinic experiences and Strong Health Geriatrics Group, Highlands at Brighton and Highlands Living Center for additional long-term care and medical directorship exposure.

Sarika Kadam  M.D.



 

Dr. Kadam completed her medical education at Byramjee Jeejeebhoy Medical College in Pune, India.  She completed her residency in Internal Medicine at Rochester General Hospital.  At Rochester General Hospital, she was involved in several committees including the ViaHealth Clinical Investigation Committee, the Palliative Care Committee and the Graduate Medical Education Committee.  She also has research experience in neuroscience and has presented numerous scientific posters on renal related disorders at national professional society meetings.

 

Renu Magu M.D.

 

Dr. Magu completed her medical education at Maulana Azad Medical College in New Delhi, India.  She completed her residency in Internal Medicine at Mount Vernon Hospital of New York Medical College and a Palliative Care Fellowship at University of Connecticut, Connecticut Hospice.  During her training, she has been actively involved in teaching medical students and residents as well as participating in committees including a Rapid Response Team Committee and Clinical Competency Committee.
Naveed Shahid M.D.


Dr. Shahid completed his medical education at Chandka Medical College in Pakistan.   He completed his residency in Internal Medicine at Nassau University Medical Center in East Meadow, New York.  He also has prior research fellowship training in Pulmonary Disease and Critical Care Medicine.  He was a member of the Infection Control Committee at Nassau University Medical Center and the recipient of the Best Graduate Award.
Hassan Shahrayer Sheik M.D.

 

 

Dr. Sheik completed his medical education Aga Khan University in Pakistan.  He completed his residency training in Internal Medicine at Penn State Hershey Medical Center in Hershey, Pennsylvania.  Dr. Sheik has prior research experience in both Nephrology and Pulmonary & Critical Care Medicine.  As an ambulatory medicine resident, he completed a research project involving clinical performance measures to track quality improvement in hypertension management in outpatients.   He has had poster presentations at the Pennsylvania College of Internal Medicine.

 
Julie Yoon M.D.

Dr. Yoon completed her undergraduate education at Cornell University (B.A.) and Upstate Medical University in Syracuse, New York (B.S. in Medical Technology).  She completed an M.S. in Microbiology at Washington State University prior to her MD at Upstate Medical Center in Syracuse, New York.  She has extensive professional experience in Microbiology including publications and a teaching award.  Dr. Yoon joined our fellowship in 9/07, will complete her training in 10/08 and remain in the Rochester community.
