We understand that you can't plan when you'll get sick, so we try to keep our appointment times flexible to meet your needs. We will do everything possible to schedule a sick appointment on the same day you call. Sometimes this may mean that you see someone other than your regular doctor, but our team approach to your health care makes it possible for us to work together to meet your scheduling requirements.
Our physicians see patients by appointment. You can schedule an appointment between 8:30 a.m.–noon or between 1:00–4:30 p.m., Monday through Friday. We try to stay on schedule, but ask for your patience and understanding when an unavoidable delay occurs. To help us stay on schedule, please arrive on time for your appointment and have your medical records transferred to our office before your first visit. If it is your first appointment, please arrive 15 minutes before your scheduled appointment to allow for registration.
If you are unable to keep your appointment, please call to cancel. Failure to cancel an appointment denies other patients the opportunity to see our physicians.
If you are a new patient, your new doctors will review your health history at your first visit. It would be helpful to have any of your records transferred to our office before your visit, especially shot records.

If you need to be seen for an emergency which cannot wait until regular office hours, please call the after-hours answering service at (585) 654-5432 for assistance. We are open Wednesday evenings and Saturday mornings for emergency visits only.
Welcome to the Culver Medical Group where both adults and children can get their medical care. We believe health care is a team responsibility. The more you know, the better decisions you will make about your own health. Your doctor, nurse and social worker will work with you and the important people in your life to develop the plan of care that is best for you.
You can expect us to help you learn about your health and how to treat any diseases or illnesses you may have. We will also help you try to lower you risk for certain diseases and to inform you about recommended screening tests for your age and risk factors. We invite your participation by coming to appointments, bringing your medication, and asking questions.
There are six full-time attending doctors in the office who have their own practices and also spend time teaching younger doctors. Many of the doctors in the office are completing residency training in both internal medicine and pediatrics. These doctors have completed medical school and are now working under the supervision of attending doctors. You may see one of the residents in the office even if your insurance lists one of the attending doctors. The residents are always supervised by the attending doctors and, occasionally those attending doctors also talk with you and examine you during your visits to the office. Our residency program is one of the top internal medicine/pediatrics programs in the country and we are proud to have the excellent providers it brings to us.
Your doctor will determine when you need a regular check up as an adolescent and adult. These are longer visits to make sure you are getting the preventive care you need.
Newborns will be seen at Highland Hospital and Rochester General Hospital. Babies born at other hospitals will be seen by the staff physician hospital and we will be informed of their medical status. Please arrange a first visit to our office before leaving the hospital.
The first shots will be at the 2 month visit. By the time your child is 2 years old, he or she should have 3 DTaP, 3 Polio, 3 Hepatitis B, 4 HIB, 4 Prevnar, 1 Chicken Pox, 1 MMR, and 2 Hepatitis A shots. One shot each of DTaP, Polio, MMR, and Chicken Pox is given to children before they enter school.
Before coming to your appointment, please check to see which prescription refills you may need so we can provide them for you at the time of your visit.
We have a new policy for prescription refills; we now require 3 business days for handling your refill requests.
Most HMO insurance plans, such as Blue Choice, Preferred Care, StrongCare, Blue Choice Option and Preferred Care Option, require prior approval of all hospital emergency room visits for coverage. Please call the office at (585) 654-5432 before seeking emergency room treatment.
However, if you are experiencing a life-threatening emergency, such as difficulty breathing or serious injury, go to the nearest emergency room and the hospital staff will notify us.
The Culver Medical Group is an affiliate of URMC. Our providers admit adult patients to Highland Hospital and children to the Golisano Children's Hospital at Strong and Rochester General Hospital.
The Culver Medical Group is located on the corner of Culver Road and Main Street at 913 Culver Road. The office is across the street from McDonalds and can also be easily reached on bus routes 2, 8 and 30.
Payment is due at the time of your visit. We accept cash, personal checks, or credit cards. We also accept most types of insurance, including Blue Choice, Preferred Care, Blue Choice Option, Preferred Care Option and Medicaid. If your insurance covers office visits, we will complete the necessary forms and bill the insurance company for you.
The Culver Medical Group and URMC are committed to providing health care to everyone. Since we are affiliated with Highland Hospital, we have access to the Charity Care assistance program, which covers office visit costs. Our social worker will work with the uninsured to discuss health insurance and payment options.
Our providers are board certified in internal medicine, and, as faculty members of the University of Rochester School of Medicine and Dentistry, they teach medical students and residents. Our residency program is one of the top internal medicine/pediatrics programs in the country.
As part of the doctors' teaching responsibilities, medical students and residents may be present in the office. These residents have completed medical school and are now working under the direct supervision of our attending physicians. If you prefer not to have a student or resident involved in your appointment, please let us know and we will be happy to accomodate you.
