To schedule an appointment, call (585) 538-6250.

We do our best to make sure that you see your doctor when it's convenient for you. We offer same day appointments for patients with urgent problems.

Your cooperation in arriving on time for your appointment helps us stay on schedule. If you need to cancel your appointment, please call us. When you notify us, we can offer your appointment time to our other patients. After three missed appointments, you will be charged a missed appointment fee of $50.

First time patients should arrive 15 minutes early for their scheduled appointment to allow for registration. To save time, fill out our new patient registration form and bring it with you.

Before your first appointment, call your doctor's office and have your records forwarded to our office. You will also need to contact your insurance company to change your primary care provider.

We also understand that listening should begin well before the exam room. That’s why they have reassigned staff members, added more phone lines and have made a dedicated effort to accommodate each and every patient’s needs. At Caledonia Family Medicine, we ’ll start listening the minute you call.

Prescription Refills

Routine prescription refills are available by telephone during regular office hours, or you may bring your medications with you to your appointment. Please allow up to 24 hours for the refill to be called into your pharmacy.

Emergencies and Hospitalization

If you are experiencing a life-threatening emergency, call 911 or go to the nearest emergency room and have the hospital notify our office or the physician on call. Most HMO insurance plans require prior approval of all emergency room visits. Please call our office at (585) 538-6250 before obtaining these services. We admit patients to Highland Hospital.

Directions to Our Office

  • Take 390 S to Exit 10 (Avon).
  • Turn right on 5 & 20 at the top of the ramp.
  • Drive through Avon and continue 7 miles to Caledonia.
  • Caledonia Medical Center is on right.

Billing and Insurance

Payment is due at the time of your visit. We accept cash, personal checks, or credit cards. We also accept most types of insurance, including Blue Choice, Preferred Care, and Medicare and Medicaid. If your insurance covers office visits, we will complete the necessary forms and bill the insurance company for you. There is a $5 billing fee if you do not pay your copay at the time of your visit.
