Discharge Planning

Your attending physicians will work together with the nursing staff and various health specialists to plan for your discharge from the hospital and a continuation of appropriate medical care. Various members of this team will meet with you during your hospital stay and review information about your post-hospital care in detail.

On the day of discharge, plan to leave the hospital by 10:00 a.m. (although sometimes your discharge may be later). This helps us prepare for other patients who will be admitted later that day.

Instructions for the person who will pick you up

If you don't believe you are medically ready to leave the hospital, you have the right to appeal. Ask your nurse for information on how to appeal the decision to discharge.

Before You Leave

Make sure you understand your treatment plan, medications and/or supplies. Here is a checklist that may help as you anticipate your discharge.
