What to Bring to Your Pre-Admission Visit

  • Pre-admission registration form. Print this form out now, fill it out and bring it with you to your appointment.

  • Health screening form. You will need to complete this form before your appointment.

  • If your doctor gives you a form listing the tests you need to have, please bring it with you. More about pre-admission tests.

  • Bring your insurance cards with you to your pre-admission visit and on the day of your procedure, because we will need to make a copy of your card. You may want to check with your insurance company before your procedure to ensure you've obtained any pre-admission testing authorizations that may be required.

  • If you will be admitted to the hospital after your procedure and you have a Living Will, Health Care Proxy, or Power of Attorney, bring a copy so we can add this to your medical record.

  • Bring a list of questions that you may have about your procedure. Our staff will help answer your questions and will provide you with pre-procedure instructions.

  • IMPORTANT: If you didn't sign the consent form that you received from your doctor, you will need to sign it and bring it with you to your appointment. We need your signed consent form before you have your procedure.

Please note: You can eat and take your medications as usual when you come to your pre-admission appointment, unless otherwise directed by your doctor.
